If Barbara had only just met the Doctor, or hardly knew who he was, she'd be positively terrified. The mix of suddenly ducking out of the way of general sight, and his manic grin which made his eyes twinkle with potentially dangerous mischief, was a little unnerving.
But she knew the Doctor well enough to know that nothing terrible was going to happen.
